ICD-11 vs ICD-10: What South African Healthcare Professionals Need to Know

Understanding the Shift from ICD-10 to ICD-11

The International Classification of Diseases (ICD) is the global standard for classifying diseases, injuries, and causes of death. Published by the World Health Organization (WHO), the ICD underpins clinical documentation, medical billing, health statistics, and resource allocation across the world — including South Africa.

South Africa has long used ICD-10 as its primary classification system. However, the WHO released ICD-11 in 2022, and countries around the world are in various stages of transition. For South African healthcare professionals — especially those involved in clinical coding, billing, and documentation — understanding what is changing and why it matters is urgent.

What is ICD-11?

ICD-11 is the 11th revision of the International Classification of Diseases. It was officially adopted by WHO Member States in 2019 and came into effect on 1 January 2022. ICD-11 represents the most significant overhaul of the classification system in over 30 years. It is entirely digital-first, more detailed, and incorporates modern medical knowledge including genetics, traditional medicine, and new disease categories.

Key Differences Between ICD-10 and ICD-11

1. Structure and Detail

ICD-10 contains approximately 14,400 codes. ICD-11 contains over 55,000 codes — nearly four times as many. This increased granularity allows for far more precise clinical documentation. For medical coders, this means learning an entirely new coding logic and structure.

2. Alphanumeric Format and Post-Coordination

ICD-10 uses an alphanumeric format (e.g., J18.9 for Pneumonia, unspecified). ICD-11 uses a different alphanumeric stem-and-extension system that allows for post-coordination — meaning coders can combine multiple codes to describe complex clinical scenarios with greater accuracy.

3. Digital Integration

ICD-11 was designed for digital health systems from the ground up. It integrates with electronic health records (EHR), clinical decision support tools, and is available in multiple languages. This makes it better suited for South Africa's growing digital health infrastructure.

4. New Disease Categories

ICD-11 includes entirely new chapters and disease categories that did not exist in ICD-10, including a dedicated chapter for conditions related to sexual health, a chapter on disorders of bodily distress, and the inclusion of traditional medicine diagnoses. Conditions like COVID-19 can be coded accurately within ICD-11's framework.

South Africa's Transition to ICD-11

The South African Department of Health (DoH) and the Council for Medical Schemes (CMS) are actively working towards the national adoption of ICD-11. While a firm legislative deadline for the full switch has not yet been gazetted, the transition is underway, and private medical schemes and hospital groups are already preparing their systems and staff.

Medical billing software providers in South Africa are updating their platforms, and facilities that delay training risk billing errors, claim rejections, and compliance issues when the transition is enforced.

Who is Affected?

The transition to ICD-11 affects medical coders and clinical coding specialists, medical billing practitioners and practice managers, clinicians responsible for documenting diagnoses, health information managers and data analysts, and hospital administrators and compliance officers.

Why Training is Urgent

The complexity of ICD-11 means that transitioning without proper training is risky. Incorrect coding leads to claim rejections, underpayment, audit exposure, and compromised patient data. Facilities that invest in ICD-11 training now will be ahead of the curve when the formal transition arrives.
